Patient Navigator | University of California San Francisco - Military Veterans

Reporting to the Director of Executive Health Practice, the Clinical Patient Navigator (RN) is responsible for coordinating and delivering high-touch, concierge-level patient care within a specialized internal medicine primary care practice. This role plays a pivotal part in ensuring seamless access to institutional services for a unique and high-profile patient population, including UCSF Health major donors, politicians, UC senior leadership, and faculty. The RN Patient Navigator works in close collaboration with physician leadership and the Director of Executive Health Services to uphold and enhance the exemplary patient experience expected in this practice. Responsibilities include: * Serving as a central liaison between senior administration, faculty and community physicians, hospital departments, and patients/families to facilitate streamlined access to services. * Overseeing key aspects of day-to-day primary care operations within the clinic * Coordinating second-opinion consultations for both concierge and international patients * Upholding exceptional customer service and communication standards, ensuring all interactions reflect diplomacy, grace under pressure, kindness, and compassion, even in highstakes, time-sensitive situations. As a licensed Registered Nurse (RN), this individual must maintain expertise in clinical best practices, regulatory compliance, and infection control standards, ensuring the Executive Health Practice is in a continuous state of readiness for regulatory inspections. The RN Patient Navigator is also expected to stay current with evolving ambulatory care policies and procedures, guaranteeing a safe, efficient, and patient-centered clinical environment. The final salary and offer components are subject to additional approvals based on UC policy. Your placement within the salary range is dependent on a number of factors including your work experience and internal equity within this position classification at UCSF. For positions that are represented by a labor union, placement within the salary range will be guided by the rules in the collective bargaining agreement. The salary range for this position is $129,600 - $303,800 (Annual Rate).
