The Registered Nurse in Home and Community Based Services provides and directs provisions of nursing and personal care to patients in their homes as prescribed by the physician or as authorized by the state/program and in compliance with applicable laws, regulations, and agency policies. The Registered Nurse coordinates the plan of care with other health care professionals involved in care and helps to achieve and maintain continuity of patient care by planning and exchanging information with physician, agency personnel, patient, family, case manager and other community resources.

 

Responsibilities




  • Provides high quality clinical services within the scope of practice and within infection control standards, in accordance with the plan of care/service plan, and in coordination with other members of the patient/client’s care team from admit through discharge.


  • Completes clinical nursing assessments in accordance with federal and/or state program requirements and as required by payer.


  • Ensures the patient/client’s eligibility and medical necessity for services as defined by payer source and agency policy.


  • Develops and revises individualized plans of care and/or service plans according to federal and/or state program requirements with other community providers.


  • Ensures plan of care frequency and duration meets the needs of the patient and initiates plan of care revisions as needed with physician approval.


  • Provides oversight and supervision of the plan of care/service plan as required by agency policy, federal, and/or state program and as required by payer including services provided by the licensed practical nurse and paraprofessional, for assigned caseload.


  • Communicates assessment findings and performs care coordination to the physician, case manager, and/or other appropriate members of the patient/client’s care team.








  • Reports clinical findings including changes in patient condition to the clinical manager/supervisor and physician timely.


  • Demonstrates flexibility with schedule to meet the changing needs of assigned patients on caseload.


  • Administers medications per physician order and in accordance with federal, state, or local regulations.


  • Transcribes physician orders accurately and timely.


  • Participates in and contributes to patient case conferences with all disciplines of the patient’s care team.


  • Maintains communication with agency case coordinator regarding patients’ schedule and service authorization as needed.


  • Reports infections and incidents according to the agency policy.


  • Maintains compliance with documentation requirements, accurately and thoroughly documents patient visits per policy, and signs and submits in the EMR timely.


  • Maintains compliance with the agency’s electronic visit verification process.


  • Attends staff meetings as scheduled by agency.


  • Participates in the RN on-call rotation for the agency.


  • Completes all orientation and annual training timely as required by the organization and according to federal, state, or local regulations.


  • Assists with the orientation and/or training of agency staff as needed.


  • Performs competency evaluations/assessments for agency staff as assigned.


  • Participates in the agency’s performance improvement process.


  • Adheres to all policies and procedures of the organization.


  • Performs all other duties assigned by Executive Director/Supervisor.




Education and Experience

Experience Desired A minimum of one year experience as an RN preferred License Requirements





  • Must have current RN licensure in state of practice.


  • Current drivers license, vehicle insurance and access to a dependable vehicle or public transportation.


  • Current CPR certification required
