Position Summary:
Rutgers The State University is seeking a Licensed Practical Nurse ( LPN ) in the Community Based Services of Rutgers University Behavioral Healthcare.
The primary purpose of the Licensed Practical Nurse ( LPN ) position is to provide medical-focused support services, including wellness education and support, to individuals with mental health and substance use disorder diagnoses, in their homes and other community-based settings in accordance with all relevant program requirements.
Among the key duties of this position are the following:
- Provides medical-focused services and intervention to individuals.
- Participates in data collection related to the individuals' status and needs.
- Provides input to into the plan of care by communicating information pertinent to the individual's status.
- Provides direct service within scope of practice to ensure implementation of the plan of care.
- Visits individuals in the community to ensure their understanding of their medical condition(s) and recommended course of treatment.
- Monitors medical condition within established guidelines, and documents in accordance with UBHC and all regulatory requirements.
- Informs and instructs individuals and where relevant significant others on the individual's health/illness state and plan of care.

PHYSICAL DEMANDS:
- Ability to speak clearly and write proficiently.
- The ability to focus on the task at hand despite numerous interruptions.
- Must be able to sit for extended periods of time and document into an electronic medical record while in the field.
- The employee spends much time driving, walking, standing, bending and talking with individuals served in the community located throughout the region.
- Transport of individuals served in state vehicles.
- The employee can expect to lift up to 20 pounds.
WORK ENVIRONMENT:
- The work environment is predominantly in the community in individuals' homes, shelters, soup kitchens and places where homeless individuals congregate (i.e. forests, train stations, under bridges etc.).
- Individual wellness checks and crisis intervention required.
- Subject to unpredictable situations, crises, to include behavioral, medical, substance use and psychiatric.
- Occasionally exposed to individuals exhibiting assaultive behaviors.
- Exposure to blood borne pathogens that requires use of personal protective equipment.
