Licensed Practical Nurse Home Visits - Military Veterans

Summary
We are looking to hire a LPN for Home Visits in the Waycross, GA area. We offer flexible schedules, benefits for those who qualify, and weekly pay.
The Home and Community-Based Services Licensed Practical/Vocational Nurse is responsible for: (1) providing direct nursing care as prescribed by the physician to acutely ill, and/or chronically ill patients in their homes; (2) recording clinical and progress notes; and (3) coordinating other patient care activities/services to provide the highest quality of patient care in the geographic service area to which she/he is assigned. She/he works under the direct supervision of the Registered Nurse and/or designated supervisor.
Nursing activities are limited to those in compliance with the State LPN/LVN Practice Act, any applicable licensure/certification requirements and agency policies and procedures.
Responsibilities
- Provides direct skilled nursing care under the supervision of a Registered Nurse and in accordance with the plan of care.
- Utilizes the E.V.V. (Electronic Visit Verification) program as applicable. Records clinical and progress notes at the time of the visit to ensure continuity of qualitative patient care in a timely basis.
- Prepares equipment and material for use according to the plan of care, practicing appropriate procedure technique, including Administers medications as prescribed by a physician and permitted by law.
- Assists with teaching the patient and/or family members regarding the patient care needs and other related problems of the patient at home.
- Ensures confidentiality of patient records/information.
- Attends team meetings and staff meetings to enhance team communication and coordination of quality services
- Observes, records, and reports (1) signs and symptoms; (2) reactions to treatments and medications; and (3) any change in the patient’s physical or emotional condition in a timely matter to the Registered Nurse and physician as applicable.
- Participates in the organization’s Performance Improvement and Compliance programs and adheres to policy’s and procedures.

Education and Experience
Education Requirements
- Must meet educational/experience requirements outlined in the laws of licensure states.
- A minimum of one (1) year of acute care experience preferred; previous home care experience preferred.
- Must have current LPN licensure in state of practice.
- Current driver’s license, vehicle insurance and access to a dependable vehicle or public transportation.
- Current CPR certification required.

- Must possess strong, patient management, and documentation skills.
- Must possess the disposition and personality to work harmoniously with a wide variety of people and all levels of staff, and interact professionally as a member of a multi-disciplinary team.
