The Patient Navigator for the Family Integrated health Care Transitions (FLIGHT) Program serves as a non-clinical resource and liaison for patients and their families and assists with navigating all aspects of the healthcare system. Provides non-clinical education in regards to treatment and liaisons with the patientAEs clinical service providers to address specific concerns. May have in-depth specialty expertise and focus. The Patient Navigator is primarily responsible for representing the administrative team as the public face of the Practice and works closely with the administrative, clinical and management teams to support practice operations and customer service recovery and intervention efforts. S/he provides individualized assistance and support to patients, serve as a resource and advocate for pediatric patients and families. In addition, s/he provides support to all functions of the administrative teams including but not limited to: CRM messages, telephone encounters, referrals, APeX in-baskets, scanning, filing, authorizations, and billing. The position requires self-initiative, as well as exceptional social skills and emotional intelligence in order to support patients with a wide variety of needs and the ability to effectively communicate with faculty and staff throughout the clinical enterprise. This position is a vital and integral part of the patient-centric case mission and direct patient education efforts for complex patients. The goal is to ensure patients have full, equitable, and convenient access to the clinical services they require and to the health education needed to make informed decisions about their care. This position makes a difference for patients in an outpatient care unit by providing excellent customer service, facilitating and ensuring the accuracy of the information flow between medical, hospital staff and departments to maximize unit efficiency and transitions across settings. The Patient Navigator is responsible for the maintenance of all routine clerical operations and communications. S/he adheres to the UCSF House and Telephone Standards and is sensitive to the needs of patients, staff and providers at all times. The Patient Navigator is a team player who works closely with others and who is flexible in dealing with the changing priorities. S/he is a self-reliant individual who synthesizes his/her knowledge of practice operations in order to problem-solve, prioritize and facilitate complex transactions in the course of his/her daily activities. S/he will play a key role in ensuring that accurate and actionable data is reported to both internal and external stakeholders. Through analysis and collaboration with analytic and clinical teams, the Patient Navigator will help to demonstrate UCSF's ability to improve population health clinical outcomes and healthcare utilization. he final salary and offer components are subject to additional approvals based on UC policy. Your placement within the salary range is dependent on a number of factors including your work experience and internal equity within this position classification at UCSF. For positions that are represented by a labor union, placement within the salary range will be guided by the rules in the collective bargaining agreement. The salary range for this position is $55,200 - $95,900 (Annual Rate).
