With minimal supervision, the Medical Social Consultant works with children and adults with developmental disabilities and their families to get the support and services they need. This includes completing diagnostic evaluations, intakes, and therapy. The position is part of a multi-disciplinary team that works holistically with people of all ages and provides services in both English and Spanish. Duties & Responsibilities: Complete psychosocial assessments: these assessments provide information about a person's level of cognitive, academic, social-adaptive, and emotional functioning. Screen calls from families and other referral sources and complete the intake process by performing an initial assessment and determining eligibility for the Family Clinic program based on residency and potential developmental disability, with or without mental health concerns or symptoms. In-person intakes may also be required. Provide Psychotherapy treatment, individual and group, as appropriate for each case; Formulate pre- and post-treatment plans and summaries: Follow up with clients and families post-therapy to check on clients' progress and further needs. Maintain caseload appropriate for the complexity of cases. Participate in collaborative programs with outside agencies; conduct internal and external training; provide training to professionals, families, and students on developmental disabilities; develop relevant training materials: coordinate parent and children support groups. Consult with members of the care coordination team regarding areas of social and emotional issues and problems, and for overall coordination activities; Interact with members on clinical teams in evaluating the most effective intervention. Organize and analyze intake data to determine efficacy of the selected service intervention; Create demographic reports on clients and quarterly data reports; Compile quarterly data. Perform other related duties and participate in special projects as assigned. Minimum Qualifications Required: Master's degree from an accredited school or university in social or behavioral science or a related health specialty area. Based on institutional requirements for each position: Current State of IL Licensure as a Licensed Social Worker or a Licensed Clinical Social Worker. Preferred Qualifications: Early Intervention Certification Bilingual language skills with the ability to read, write, speak, and translate Spanish and English. Demonstrated knowledge of developmental disabilities (including autism), community resources, and mental health issues.
