Psychiatric Nurse - Military Veterans

We are seeking a compassionate and dedicated Registered Nurse (RN) or LPN to join our team and provide specialized care to clients with serious mental illness who require higher levels of community care. This role will focus on treatment, rehabilitation, and support for individuals in need of intensive services to manage their mental health.

Key Responsibilities:

- Conduct in-home visits to administer prescribed medications, including oral medications and injections.
- Provide transportation and accompany clients to medical appointments.
- Assess health, social, psychiatric, and mental health needs of clients.
- Assist in developing personalized strategies to meet clients' healthcare needs.
- Engage in health promotion, prevention, and educational activities for clients.
- Ensure that patients are seen according to their medical and psychiatric needs, coordinating necessary appointments.
- Complete clinical documentation using collaborative documentation, ensuring compliance with SCDMH and QA requirements.
- Participate in clinical supervision and daily treatment team meetings to ensure coordinated care and best practices.

Qualifications:

- Graduation from an accredited school of nursing.
- Licensed as a Registered Nurse by the SC Board of Nursing or an LPN.
- A minimum of two years of experience in a clinical setting is preferred.
- At least one year of experience working with adults diagnosed with serious mental illness is preferred.
- Working knowledge of psychiatric medications is a plus.
- Strong communication, organizational, and documentation skills.
- Ability to work independently and as part of a multidisciplinary team.
- Compassionate and patient-focused approach to care.
