MDS Coordinator, RN (SNF) - Military Veterans

We are looking for a MDS Coordinator to work in a beautiful 160 bed family-owned and operated skilled nursing facility that specializes in short-term, long-term and dementia care.

Job Details:

As the MDS Coordinator you will collaborate with members of our health care team to assist patients in achieving established outcomes of care.In this key role, you will be responsible for coordinating and overseeing the MDS (Minimum Data Set) process to ensure accurate and timely assessments of residents. You will work closely with interdisciplinary teams, including physicians, therapists, and social workers, to develop individualized care plans that meet the needs of our residents.

Key Responsibilities:

Complete comprehensive MDS assessments in accordance with state and federal regulations.
Ensure that all assessments are accurate, timely, and in compliance with CMS (Centers for Medicare & Medicaid Services) requirements.
Work with the interdisciplinary care team to develop and review individualized care plans for each resident.
Monitor and track resident progress to ensure quality care is delivered and documentation is maintained. Provide education and guidance to staff regarding MDS processes and documentation standards. Ensure timely submission of all MDS assessments and care plans to appropriate regulatory agencies. Participate in care conferences, quality assurance meetings, and other team-based activities. Review and update resident data as needed, ensuring that all required information is properly documented. Monitor for changes in residents’ conditions and communicate necessary adjustments to care plans.

Qualifications:

Registered Nurse (RN) with an active and valid license in Connecticut.
Minimum of 2 years of experience as a Registered Nurse in a skilled nursing or long-term care setting.
Previous experience with MDS assessments and care planning is required.
In-depth knowledge of federal and state regulations governing skilled nursing facilities.
Strong attention to detail, organizational skills, and ability to prioritize tasks.
Excellent communication skills, both verbal and written.
