What You’ll Do:

- Make a difference: Evaluate and monitor the quality of care for residents, helping them achieve their care goals
- Be a key player: Collaborate with a dedicated healthcare team to provide exceptional care
- Lead assessments: Oversee and manage the development and completion of resident assessments (MDS)
- Drive positive outcomes: Support the creation of care plans that help residents thrive.

All You Need:

- A current Connecticut RN license
- Experience in MDS coordination, care planning, case mix index, and compliance
- Previous charge nursing experience is a plus!
- A passion for maintaining high-quality standards, including a CMI greater than 1 at 90% or better
- Excellent attention to detail in submitting error-free assessments
