Position Summary:
We have an exciting opportunity to join our team as a CTC Manager, Comprehensive Transfer Center.

In this role, the successful candidate Provides health system leadership for Comprehensive Transfer Center CTC managing all Transfer Center activity for NYU Langone Health inclusive of current and future scope Manages transfers and direct admissions into and within NYU Langone Health in collaboration with CTC leadership CTC Medical Directors Hospital Chief Medical Officers CMOs frontline staff and members of the interdisciplinary teams Reports to Director of CTC and Patient Flow Operations

Job Responsibilities:

  • Manages all Transfer Center functions including direct oversight of all direct admissions transfers and patient flow activity into and within NYU Langone Health
  • Manages CTC Assistant Manager Transfer Center Nurses and Flow Coordinators to ensure direct admissions and transfers take place in safe and efficient manner and in accordance with approved protocols and policies
  • Provides leadership for the CTC serving as a role model and resource for all staff Provides off hour support for CTC escalations including those requiring senior leadership review and/or approval
  • Responds to all stakeholders in a professional manner with the aid of the Electronic Health Record EHR approved clinical protocols and established patient care protocols and healthcare information
  • Collaborates with clinical and unit based leadership to develop protocols and service level agreements
  • Facilitates transfers direct admissions and patient flow activity throughout the continuum of care by leveraging systematic processing of patient acuity and expert critical thinking and decision making
  • Acts as liaison between CTC and key stakeholder departments including Patient Access Revenue Cycle Operations Care Management Social Work Regulatory etc
  • Serves as CTC team lead for partnership with external Emergency Medical Service EMS contracted services Collaborates on development of workflows and associated policies to ensure safe and reliable transport of patients
  • Ensures Transfer Center Nurses and Flow Coordinators communicate clearly and document all pertinent patient information in the EHR providing an accurate picture of the patients clinical condition for accepting attending physicians nurses and other clinicians
  • Oversees data collection monitoring evaluation and analyzing of transfer or direct admission issues for the purpose of developing and implementing protocols policies and procedures
  • Manages and implements interdisciplinary quality improvement initiatives developing new workflows to support the changing needs of the organization
  • Identifies and addresses learning needs of all Transfer Center Nurses Flow Coordinators and other CTC staff members Develops training materials for frontline staff department leadership and other stakeholder departments
  • Oversees execution of departmental policies procedures and communications to ensure adherence to appropriate processes
  • Models excellent customer service and relationship skills including in person telephone and email communication with key internal and external stakeholders
  • Serves as CTC team lead to ensure strong communication between all team members external stakeholders and senior leadership Ensures a collaborative interdisciplinary approach to achieve optimal outcomes and further the goals of the CTC
  • Perform other duties as assigned

Minimum Qualifications:
To qualify you must have a Bachelors degree 3 5 years of relevant experience Current unrestricted New York State RN license Strong problem solving multi tasking analytical/critical thinking and conflict resolution skills Ability to establish and maintain collaborative and effective working relationships Ability to communicate effectively in oral written and electronic formats i e email electronic medical record etc Excellent telephone manner and use of professional vocabulary Ability to function calmly in a stressful situations and fast paced environment Positive enthusiastic customer service oriented personality Proficiency in Microsoft Office suite Outlook Word Excel etc.

Preferred Licenses: Registered Nurse License-NYS

Preferred Qualifications:
BCLS MSN or equivalent degree Epic knowledge Masters Degree in health care related field

NYU Langone Health provides a salary range to comply with the New York state Law on Salary Transparency in Job Advertisements. The salary range for the role is $117,107.95 - $195,179.92 Annually. Actual salaries depend on a variety of factors, including experience, specialty, education, and hospital need. The salary range or contractual rate listed does not include bonuses/incentive, differential pay or other forms of compensation or benefits.
