Licensed Vocational Nurse - Primary Care - Military Veterans

Under the supervision of the UCSF Primary Care Administrative Nurse Manager the Licensed Vocational Nurse utilizes the nursing process to assess and identify the individual needs of patients through observation of disease symptoms, emotional state, laboratory reports, cultural and socio economic background and response to medical regimen and treatment. Additionally, s/he is required to learn and apply the Patient Centered Medical Home Standards and Ten Building Blocks of Primary Care.He/she is responsible for providing nursing care to patients in the ambulatory setting. The LVN will work as an important member of the care team, working closely with the RN and the physicians to deliver all components of nursing care will be individualized to meet the patient's physiological and psychosocial developmental level and needs. The LVN will be responsible for chart abstraction. The Licensed Vocational Nurse consistently projects a professional and courteous demeanor and shall comply with the Medical Center House and Telephone Standards. In addition,will have clinical responsibility for administration of medications/vaccines, lab result review and monitoring, treatments and procedures pertinent to pediatric & adult primary care such as ear irrigations, wound care and urethral catheterization, as well as patient education. The LVN must have flexibility to cover multiple UCSF medical center primary care clinic sites
This individual will utilize his/her good customer service and communication skills on a daily basis via phone/electronic contact, mail correspondence, and personal contact with both internal and external customers. This employee will utilize computer skills in ApeX and other UCSF computerized programs. This individual supports the standards of the policies and procedures of the practice, and is responsible for maintaining current knowledge of pharmacology and competency in modes of administration, and complies with all Ambulatory Services and Medical Center policies and procedures for infection control, safety, and administrative and clinical practice. S/he is required to adhere to the Telephone and House Standards as adopted by the Medical Center, as well as demonstrating UCSF's PRIDE values and the AIDET fundamentals of communication in all patient care and stakeholder interactions.
The final salary and offer components are subject to additional approvals based on UC policy.
Your placement within the salary range is dependent on a number of factors including your work experience and internal equity within this position classification at UCSF. For positions that are represented by a labor union, placement within the salary range will be guided by the rules in the collective bargaining agreement.
The salary range for this position is $43.00 - $53.94 (Hourly Rate).
