Responsibilities
- The Bereavement Coordinator is a qualified professional who supervises the provision of bereavement services reflective of patient and family needs. Demonstrates proficiency in providing bereavement services to persons of all races, ages, genders, sexual orientation, social class, spiritual beliefs, and mental/emotional status as evidenced by observation and EGSS scores.
- Establishes a Plan of Care that addresses bereavement needs with clear delineation of services to be provided and frequency of service delivery up to thirteen (13) months following the death of the patient.
- Demonstrates active listening and impactful suggestions to bereaved during phone and home visits within LHC-required time guidelines as evidenced by observation, Visit Notes and EGSS scores.
- Leads and coordinates at least 12 effective sessions of Grief Support Group per calendar year as evidenced by observation and EGSS scores.
- Maintains Bereavement Binder with information and attendance sheets from Memorial Services, Grief Support Groups, Holiday events, and other Hospice and Community groups and presentations.
- Demonstrates individualized and compassionate care for each bereaved as evidenced by documentation in Visit Notes, Bereavement Risk Assessment, Care Plan, and EGSS scores.
Education and Experience
Education Desired
- Bachelor's or Master's degree in Social Work, Psychology, Mental Health counseling, Psychiatric Nursing, Pastoral Counseling or the documented equivalent in education, training or experience per state regulations.
Experience Requirements
- Experience in grief and loss counseling.
Skill Requirements
- Excellent writing and presentation
- Current CPR certification
- Current driver's license, valid vehicle insurance and access to a dependable vehicle, or public transportation.
