The Care Coordinator Nurse is responsible for ensuring the provision of quality patient care in the appropriate setting through care coordination, case management, and transitions of care to different levels of care. In collaboration with the attending practitioner, the family and other members of the health care team, coordinates individualized plans of care and communicates the plan to appropriate stakeholders. Ensures high quality care is provided as efficiently and cost effectively as possible. Facilitates continuity of care by coordinating transition to post-discharge care. Practices professional nursing and promotes patient advocacy in accordance with the ANA Code of Ethics for Nurses. It is highly anticipated this role will work closely with children with special needs and chronic illnesses. “These activities are performed in a variety of settings including during inpatient stays, outpatient specialty clinic visits, emergency room visits, home, and community visits, as well as telephone and “virtual” visits.”
