Are you ready to make a difference in your community? Join us as a Transformational Services Case Manager, supporting individuals as they work toward stability, health, and self-sufficiency. Your compassion and organizational skills will help clients access vital resources and achieve their goals.

Key Responsibilities

- Support during high-stress situations: Provide assistance during client crises, including heightened emotional states or mental health challenges.
- Resource Navigation: Assist with housing, ID recovery, employment development, and behavioral health referrals.
- Client Support: Engage with clients in inclusive shelters and support individuals in their early adulthood, helping them on their journey to stability.
- Documentation: Maintain accurate case notes and use Microsoft platforms and HMIS to track interactions and services.
- Program Coordination: Relay program information and eligibility criteria, ensuring clients understand the services available to them.

Requirements

- Experience in Crisis Management: Proven ability to manage and calm situations during high-stress scenarios.
- De-escalation Techniques: Skilled in providing calm support and managing crises with empathy and professionalism.
- Experience working with vulnerable populations: Background in social work, case management, or related roles.
- Attention to Detail: Strong organizational and documentation skills for accurate tracking and communication.
- Availability for evening and weekend hours is necessary for this role.
