CASE MANAGER - Military Veterans

About Us: We are partnered with an organization that provides exceptional services to our veterans, ensuring they receive the support and resources they need to thrive.
We pride ourselves on our collaborative approach, working closely with VA personnel, community partners, and the veterans themselves to achieve optimal outcomes. Key Responsibilities:

• Assess veterans referred to the program, identifying needs and barriers in housing, employment, education/training, and financial areas.

• Coordinate care with VA personnel and community partners to secure eligibility documentation and mental and physical health services.
• Facilitate the client intake process, ensuring all program eligibility documentation is complete.

• Collaborate with clients to develop and complete Individual Action Plans
• Maintain confidential client files, ensuring compliance with organization standards. management, etc.

Qualifications:

• Bachelor’s Degree required. A degree in Social Work, Psychology, Counseling, or a related social service field is preferred.

• Two years of experience in social services or a related field preferred. • Experience facilitating support groups preferred
