Navigates all aspects of the specialized area within the healthcare system. Provides specialized clinical education in regards to treatment and liaisons with the patient's clinical service providers to address specific concerns. Has in-depth specialty expertise and focus.
Under the direction of the Manager of Care Transitions Programs and the Medical Director for Transitions and along with other Population Health Clinical Program Leadership, this Lead Nurse Coordinator will be an integral member of the Office of Population Health Care Transitions and Relay Center Team. The Lead Nurse Coordinator serves as the UCSF-wide expert on care transitions from the Emergency Department and results reporting.Recognized as a patient navigation expert, the Lead Nurse Coordinator is responsible for maintaining and updating result follow-up and care coordination workflows across UCSF Health Emergency Departments, inpatient and ambulatory clinics. He or she serves as an expert clinical resource and liaison for patients and their families.
The Lead Nurse Coordinator establishes and maintains relationships with key stakeholders, facilitating meetings with multidisciplinary providers to present optimize effectiveness and efficiency of the Relay Center. He or she serves as the team leader and key partner with UCSF Emergency Departments. The Lead Nurse Coordinator designs and implements supportive interventions for clinically or financially high risk patient populations in need of specialized care. He or she serves as the team liaison with outside stakeholders, including the CipherHealth team.
The final salary and offer components are subject to additional approvals based on UC policy.
Your placement within the salary range is dependent on a number of factors including your work experience and internal equity within this position classification at UCSF. For positions that are represented by a labor union, placement within the salary range will be guided by the rules in the collective bargaining agreement.
The salary range for this position is $113,500 - $258,100 (Annual Rate).
