*Responsibilities for Internal Candidates 

 

The Admissions Nurse is responsible for execution of the hospice admission process in compliance with federal
and state regulations. The Admissions Nurse will advocate for the patient and family to meet those stated needs
that hospice can cover and/or refer them to other resources as appropriate.

 





  • Demonstrates competency with potential patients, hospice eligibility and meeting with families in assessment of their needs.


  • Maintains confidentiality and safety of patient/client clinical records.


  • Completes assigned admissions and patient care orders in an effective and timely manner.


  • Communicates effectively with physician(s), referral sources, patient, family, and hospice interdisciplinary team regarding the new admission of a patient. Records that may be relevant to determine hospice eligibility.


  • Communicates effectively with operational, clinical and sales leadership regarding status of pending referrals.




 

 

Responsibilities

  • Completes assigned admissions in an effective and timely manner.


  • Communicates effectively with physician(s), referral sources, patient, family, and hospice interdisciplinary team regarding the new admission of a patient.


  • Responsible for communicating and documenting the clinical assessment, identified needs and other pertinent information.


  • Records and includes in the medical record any pertinent medical history and assists in obtaining medical records that may be relevant to determine hospice eligibility.


  • Assists in ordering emergency kit medications, as well as any medications that are needed immediately for new admissions (per physician order) and facilitates processing of the medication order(s) with the pharmacy provider.


  • Assists with orders and appropriate education for DME equipment, as needed.


  • Communicates effectively with operational, clinical and sales leadership regarding status of pending referrals




Education and Experience


  • Must be a licensed registered nurse in the state of operation.


  • At least 2 years of experience in a hospice, home health, or other health care service delivery system setting.


  • Current CPR certification.


  • Current Driver's License and vehicle insurance, and access to a dependable vehicle, or public transportation.


  • LA: a minimum two-years full time experience working as an RN. However, two years of full-time clinical experience in hospice care as a licensed practical nurse may be substituted for the required two years of experience as a registered nurse.


  • MS: At least one-year full-time experience as an RN. However, three (3) years full-time clinical experience in a healthcare setting as an LPN may be substituted for the one-year full time experience as an RN.


  • VA: Experience with terminally ill required.


  • AL, AR, AZ, CA, CO, FL, GA, ID, IL, IN, KS, MI, MN, MO, NC, NJ, NV, OH, OK, OR, PA, SC, TN, TX, WA, WV:


  • No additional state specific requirements.
