CDTM Pharmacist - Neurology | Boston Children's Hospital - Military Veterans

The Collaborative Drug Therapy Management (CDTM) Neurology Pharmacist partners with collaborating physicians to support the mission of providing exceptional patient care across the Health System. They serve as a liaison between patients, ambulatory, clinical and pharmacy teams, facilitating and optimizing efficient provision of drug therapy and positive patient outcomes. The CDTM Pharmacist provides consulting services to patients regarding the effective use of medications, importance of adherence, awareness of drug interactions, and may coordinate other preventive healthcare services such as immunizations. CDTM Pharmacist plays an essential role in disease state management, identifying ways to optimize medication therapy and ensuring an optimal patient experience. This CDTM Neurology Pharmacist will be responsible for: Interviewing patient to gather all relevant data, including but not limited to general health, activity status, medical history, medication history (including OTCs), and immunization history Performing comprehensive medication review and reconciliation for each patient by assessing medication regimens and making modifications to dose, frequency or route of administration to achieve desired therapeutic outcomes, providing direct pharmaceutical care in the outpatient ambulatory setting in accordance with the CDTM Protocol Performing routine patient monitoring functions as necessary in drug therapy management, including evaluation of clinical laboratory tests related to the drug therapy management for the specific disease or disease state(s) specified within the CDTM Protocol. Preparing written notes in a consistent manner on all patients seen. Documenting all interventions, progress notes and other relevant information Working collaboratively with the care team to develop the patient care plan, promoting adherence to treatment plan and encourage appropriate follow-up for optimal medical management. Communicating any changes or adjustments in therapy to patients and caregivers in a clear and concise manner Performing follow-up phone calls to patient, patient's caregiver and/or pharmacy Monitoring the patient's response to therapy, including safety and effectiveness of medication. Presenting drug information to patients, physicians, nurses, and other pharmacists, modifying terminology and approach based on target audience Providing patient education on all medication therapy the patient is receiving and also assists patients and providers with medication access issues to ensure understanding of the care plan Establishing and measuring patient outcomes against goals and objectives Ensuring results are defined and measurable, focusing on growth, improving outcomes, clinical effectiveness and patient safety To qualify, you must have: Doctor of Pharmacy and a minimum of 5 years of experience as a licensed pharmacist. Previous pharmacist experience with neurology patients highly preferred, previous pediatric pharmacist hospital experience also preferred Current license and registration by the Massachusetts Board of Registration in Pharmacy Analytical skills to collect clinical information from diverse sources, while performing various analyses, and summarizing the information and data in order to solve patient care problems or design relatively complex plan of care for patients in a team setting MA Controlled Substance License and DEA License must be obtained within 6 month of hire - BCH will reimburse cost Schedule: M - F day shifts Boston Children?s Hospital offers competitive compensation and unmatched benefits including flexible schedules, affordable health, vision and dental insurance, childcare and student loan subsidies, generous levels of time off, 403(b) Retirement Savings plan, Pension, Tuition and certain License and Certification Reimbursement, cell phone plan discounts and discounted rates on T-passes.
