RN Care Coordinator (LTS) - Military Veterans

Summary
The LTS Care Coordinator (RN) conducts assessments, develops care plans, and provides ongoing monitoring of long term care insurance claimants.
Responsibilities
- Conduct telephonic/virtual assessments by collecting information about the client’s functional, cognitive and medical status
- Review assessments that have been completed by field assessors for quality, obtaining clarification as needed
- Evaluate the plan of care to determine if the appropriate care is in place to ensure safety and quality of life for the client
- Develop appropriate care plans utilizing knowledge of community resources and services, recommending services that best meet the needs of the client
- Maintain productivity standards as established by LTS
- Maintain quality standards as established by LTS
- Recognize and communicate problems/issues immediately for rapid resolution
- Uphold confidentiality standards as defined by LTS and HIPAA
- Participate in Quality Improvement activities
- Attend staff meetings, educational programs as directed
- Mentor/precept new employees
- Other duties as assigned by manager
Education and Experience
Education Requirements
- Registered Nurse graduate of an accredited college or university.
Experience Requirements
- A minimum of two years’ experience preferred; in the human services field, ideally in community based care.
- Experience in conducting social and health assessments.
Skill Requirements
- Understanding of geriatric care.
- Computer literacy required.
- Must be organized, well-spoken, polite, team oriented, patient and adaptable to the fluctuating needs of the position.
