Position Summary:
We have an exciting opportunity to join our team as a Home Health Aide- Home Care - Nassau - HHA CERTIFICATION REQUIRED! CAR / TRAVELING REQUIRED!.

In this role, the successful candidate A member of theHomeHealth Agency team providing personal care services to patients of all ages in accordance with the established plan of treatment in theirHomes under the direct supervision of aHome care nurse

Job Responsibilities:


    • Performs other duties as assigned

    • Provides personal care andor assists patients of all ages according to their abilities and the plan of care established by the Home Care Nurse via the following activitites: hygienic care light housekeeping preparation of meals feeding exercises

    • Observes and communicates changes in patient s status or condition as directed by the Home Care Nurse Sets priorities Completes patient referrals to NYU WUH HHA patient care reports forms daily vouchers and schedules

    • Encourages maintenance of the patient s medication supplies and equipment Reports environmental or equipment safety hazards to home office patient and family with suggestions for improvement

    • Supports and complies with all departmental and hospital policies and procedures

    • Maintains caseload schedules and travels to clients homes

    • Performs patient care techniques which require manual dexterity visual inspection manual palpation auditory auscultation ; visual discrimination and physical movement of patients and equipment

Minimum Qualifications:
To qualify you must have a High School Diploma or Equivalent Certificate of completion of a state approved program offered forHomeHealth Aides Department ofHealth DOH approved training program 1 2 years Experience working as aHomeHealth Aide HHA /Personal Care Aide in aHome Care orHospital setting Excellent communication both oral and written interpersonal and organization skills Ability to negotiate architectural barriers in patient sHomes and work within the environment of care Ability to travel to patientsHomes under all weather conditions

NYU Langone Health provides a salary range to comply with the New York state Law on Salary Transparency in Job Advertisements. The salary range for the role is $40,295.67 - $42,915.09 Annually. Actual salaries depend on a variety of factors, including experience, specialty, education, and hospital need. The salary range or contractual rate listed does not include bonuses/incentive, differential pay or other forms of compensation or benefits.
