Job Summary and Qualifications

The Licensed Coordinator is responsible for oversight and management to selected processes within the Social Work Department. This position is responsible for managing the coverage of hospital wide social work duties including all patient care and case management responsibilities. This position reports to the Social Work Director and may be involved with clinical interventions as assigned and serve as a member of a multidisciplinary team in accordance with Dominion Hospital’s social work policy.

What you will do in this role:

  • Performs and/or provide Social Work Department quality assurance for the completion of psychosocial assessments within 48 hours using reports from Meditech and submitted to Director of Social Work in a timely manner.
  • Responsible for monitoring of other HBIPS and related clinical data as assigned for Social Work Department. May include generation of reports and supportive education for staff.
  • Maintains knowledge of Utilization Management functions and performs monitoring and support as needed to social work processes.
  • Responsible for inpatient assignments to clinical therapists in a timely manner.
  • Responsible for coverage and assignments for individual patient needs and groups as needed for inpatient and PRN social work staff.
  • Coordinates and supervises the weekend calendar of staff and social work services.
  • Coordinates and may supervise students/interns.
  • Interfaces and acts as liaison with community Behavioral Health and referral agencies as designated and assigned by the Director of Social Work.
  • Serves as resource specialist and coordinator of “in-house” resources and processes.
  • Develops and maintains resource binder of resources for discharge support. Serves as liaison to organizations to maintain current information and communication to social work staff.
  • May provide support to discharge planners in discharge process.
  • Coordinated/lead Performance Improvement projects as assigned by the Director of Social Work.
  • May provide clinical support to therapeutic groups as assigned. May maintain a limited caseload as assigned by the Director of Social Work.
  • Other duties as assigned by the Director of Social Work
  • Follows work schedule as assigned: may be assigned to work one weekend day per month, if staffing and patient safety require additional staff. Social Work staff may have staggered work hours (daily hours: 8-4; 9-5).

What qualifications you will need:
  • Master’s Degree in Clinical Social Work, Counseling, or other related behavioral health field. Education should be clinically focused with study in human life span development and psychopathology.

  • LCSW or LPC
  • CPR-BLS for healthcare providers required. Crisis Prevention Interventions (CPI) required as a part of the general hospital orientation. 
  • Minimum of 2 years experience with administrative duties and supervision of staff and/or students in a health care setting; behavioral health clinical experience, including the provision of individual, family and group psychotherapy. 
  • Experience with discharge planning preferred.
