Description: The Social Worker (SW) will primarily support three county community support teams with team coordination and case management as part of a statewide community safety initiative. This is a grant-funded two-year role. The selected candidate will be an essential member of each county team to ensure timely coordination, case management support, and communication between each county team and the core project team. Duties & Responsibilities: The social worker's time will be split between each county and will be required to attend county team meetings, provide report-outs to the core project team, and assist, when necessary, with case management needs for each county. Some local travel may be required. Coordinating with county team leads to ensure teams are meeting to advance their county support teams. Coordinate meeting times and take meeting notes with each county team. Develop talking points, information briefs, and presentations as needed. Attend all required training and staffing sessions Receive, record, and transmit messages from county community support teams, communicate with community partners, respond to case management consults and referral requests to ensure the client's needs are being addressed Work in conjunction with medical care providers (e.g., case conferencing, care conference), ensuring comprehensive medical care is provided and monitored based on client individual medical and psychosocial needs Maintain confidential case notes and perform chart audits to ensure compliance with program requirements and promote successful outcomes. Assist and participate in the development of the Illinois Community Safety Committee Perform other related duties and participate in special projects as assigned Minimum Qualifications Required: 1. Master's degree from an accredited school or university in social or behavioral science or a related health specialty area. Based on institutional requirements for each position: 2. A . Current State of IL Licensure as a Licensed Social Worker (LSW) or a Licensed Clinical Social Worker (LCSW) Preferred Qualifications: A master's degree in social work, counseling or related field is preferred. Three-five years of work experience in a case management role is preferred. 1-2 years of coordination or project management experience is preferred. Community engagement experience preferred.
