Job Summary and Qualifications
Provides pharmaceutical care through active participation in the Pharmacy Clinical Program, with emphasis on providing optimal and cost-effective pharmaceutical care. Works with the medical staff in developing patient care standards and protocols as they relate to the utilization of pharmaceuticals within the hospital. Serves as an information and education resource of drug therapy for the hospital staff and employees.
What you will do in this role:
- Performs necessary patient therapy assessments to maximize therapeutic outcomes and minimize undesirable drug-related effects. Elements of clinical functions include:
- Monitor effects of medications on patients and make appropriate interventions. Interventions must be documented in the Meditech module, or equivalent.
- Review patient profile for appropriate drug regimen as it relates to diagnosis, dosage, allergies, and therapeutic interchange drug of choice, drug-drug interaction, and drug-food interactions.
- Review patient profile for potential adverse drug reactions.
- Perform necessary renal and/or pharmacokinetic dosing.
- Provide drug information to medical and nursing staff as requested.
- Participates in patient care rounds to provide input into drug Selection.
- Develops protocols and guidelines for use by the medical, nursing and pharmacy staff to ensure optimal medication usage (Generate pocket-guides, etc.)
- Initiates medication use evaluation (MUE) programs and utilizes the results to recommend appropriate medication therapy based on documented clinical reports in literature.
- Active participation in writing and/or evaluating drug therapy protocols.
- Evaluates non-formulary medication requests, and recommends appropriate therapeutic alternatives on the formulary, (or proposes addition to the formulary, through the P & T Committee).
- Provides drug education and counsels patients on appropriate medication usage, when necessary.
- Provides clinical pharmacy consults as requested by medical staff .
- Performs other duties as assigned.
- Assists in training of pharmacy technicians and others as needed.
What qualifications you will need:
- Graduate from an accredited College of Pharmacy, with current licensure to practice in Texas as a Pharmacist.
- ACPE Certification in sterile products is required.
- Successful completion of an accredited hospital pharmacy residency program, or 2 years of hospital-based clinical pharmacy experience.
