Job Summary and Qualifications

The Registered Nurse (RN) CM is responsible for promoting patient-centered care by coordinating the plan of care for the patient stay, managing the length of stay, ensuring appropriate resource management, and developing a safe appropriate discharge plan in collaboration with the multidisciplinary team. The RN CM facilitates the progression and transition of care using established criteria and in conjunction with the multidisciplinary team. The RN CM will coordinate activities that promote quality outcomes and patient throughput while supporting a balance of optimal care and appropriate resource utilization.

What you will do in this role:

  • You will be responsible and accountable for coordination of services through an interdisciplinary process that provides a clinical, financial, and psychosocial approach through the continuum of care.
  • You will balance individually identified patient and family needs with the efficacious and cost-effective utilization of resources.
  • You will, through concurrent clinical case management, assess patients to determine appropriateness of admission, continued hospitalization, clinical resource consumption, progressive care management, as well as appropriate level of care.
  • You will do discharge planning that will begin at the time of (or prior to) admission, and reassessed ongoing throughout the course of hospitalization.
  • You will monitor Quality and Risk Management issues and education needs that will also be monitored for and reported to the QRM DEPARTMENT as a part of the CQI process. 

    What qualifications you will need:

    • Current licensure in the State of Colorado as a Registered Nurse, or current active multistate Registered Nurse license,
    • Current BLS certification through the American Heart Association or American Red Cross.
    • Special certification in case management preferred.
    • Associates Degree or Nursing Diploma. BSN Preferred.
    • 2+ years’ experience in case management OR 3+ years’ experience in clinical nursing required.
    • InterQual experience preferred.
    • One or more years Case Management experience preferred
    • Ability to assess information, apply critical thinking processes to determine course of action, define expected outcomes and evaluate effectiveness of plan through continuous assessment of the patients progress.
    • Ability to compile, evaluate and report statistics to members of the team and utilize the information to facilitate process improvement activities.
    • Strong oral/written communication skills are necessary for meeting facilitation, educational presentations, team collaboration and patient interviews.
    • Requires excellent human relations skills and strong analytical/organizational skills.
    • Must have ability to perform multiple tasks simultaneously and work in high stress environment.
    • Position requires flexibility and cross coverage.
