Are you looking for an opportunity to make an impact on heart transplant patients? Our world-renowned team at Barnes-Jewish Hospital is looking for a Clinical Pharmacist Specialist, Solid Organ Transplant with a focus in heart transplant.

 

In this role, you will provide care to heart transplant and advanced heart failure patients. Additionally, you will work with left ventricular assist device (LVAD) to improve patients quality of life. 

 

PGY1 residency required. PGY2 in solid organ transplant or cardiology preferred.

The pharmacy at Barnes-Jewish uses an integrated practice model with more unit-based positions. Here, department staff has access to a number of drug information resources, including many online references accessible from department computers. An on-site drug information center provides support for more in-depth questions.


Preferred Qualifications

Role Purpose

Evaluates and recommends appropriate drug therapy to achieve optimal cost-effective patient outcomes, by working directly with physicians, other health professionals, and patients. Participates in the development, provision and evaluation of comprehensive pharmaceutical care and quality assurance programs through scholarly activities. Serves as a resource to the pharmacy staff and other hospital departments.

 

Responsibilities

  • Assess the overall status of the patient’s health condition and evaluates medication profile for optimal patient outcomes. Monitors therapy and goals of care. Documents all activities and communication as appropriate.
  • Participates in professional development and research activities.
  • Contributes to performance improvement, clinical effectiveness, drug use evaluation, and HSO/BJC committee activities.
  • Participates in the education and training of pharmacy staff, residents and students, and serves as a role model and mentor.
  • Participates in the identification and implementation of targeted drug cost reduction programs.
