JOB SUMMARY

The Palliative Care Chaplain provides spiritual / pastoral / liturgical / sacramental ministry in an inpatient hospital setting and an outpatient clinic setting when needed. The Chaplain is a member of the clinical and academic interdisciplinary team and provides spiritual and emotional care to patients and their families. Additionally, the Chaplain provides support to colleagues by promoting wellness and mitigating the effects of compassion fatigue. The Chaplain also serves as a faculty member of the ACGME Accredited Hospice & Palliative Medicine Fellowship Program at NCH and provides spiritual leadership and education in collaboration with our community hospice Chaplains (AVOW and VITAS) during the Spirituality fellowship core rotation. In addition, the Chaplain provides regular spiritual education to our NCH fellows, resident physicians, medical students, nurses and community. The Palliative Care Chaplain is encouraged to take an active role in participating in the weekly education and interdisciplinary team building activities, as well as research.

ESSENTIAL DUTIES AND RESPONSIBILITIES

• Provides pastoral care that meets the spiritual and emotional needs of patients, patients' families, medical staff and volunteers.

• Acknowledges and respects stakeholders' beliefs, culture and values related to life's meaning, including suffering and loss and the desire to receive support.

• Understands principles of spiritual care and the value of within the healthcare continuum.

• Demonstrates advanced knowledge and skills related to adjustment to condition, goals of care, advanced care planning, grief and bereavement care.

• Demonstrates a knowledge of theology, spirituality, religions and religious diversity.

• Provides a spiritual assessment of the patient/family/caregivers; develops a comprehensive spiritual care plan in conjunction with the interdisciplinary team, and overall care plan.

• Strives to reduce patients' and families' anxiety and spiritual distress by facilitating spiritual comfort, healing and support.

• Documents all patient/family/caregiver contacts; promotes coordination amongst the interdisciplinary team and professionals involved in the patient's care plan.

• Facilitates quarterly Palliative Care Spirituality Meetings inviting all hospital chaplains in Spiritual Care, as well as Palliative Care Team Members to foster ongoing collaboration with the spiritual care of our hospital patients and increase awareness of the diversity of religions, beliefs, rituals and practices of those in our community.

• Promotes and supports an interdisciplinary approach to pastoral care of seriously ill patients, including those who are terminally ill and dying.

• Serves on institutional committees as appointed and as voluntarily elected.

• Promotes a positive & effective working relationship with patients/families, physicians, healthcare professionals, community faith-based organizations and members of the interdisciplinary team.

• Participates in community outreach programs, civic organizations and clergy groups to enhance the vision, mission and visibility of the pastoral care of seriously ill patients.

• Coordinates volunteer Palliative Care Chaplains visits and assists in the education of the Palliative Care Volunteers

• Assists with Palliative Care Program Development by closely following the CAPC Website (Center for Advancement of Palliative Care) for opportunities to further develop the NCH Palliative Care Program, opportunities to participate in upcoming program competitions, relevant webinars, research, etc.

EDUCATION, EXPERIENCE AND QUALIFICATIONS

• Bachelor's Degree and Graduate Theological Degree, preferably Master of Divinity, required.

• 2 Years' Experience in Spiritual Care.

• 4 Units, 1600 hours of Clinical Pastoral Education through ACPE.

• Board Certified via APC or must obtain board certification within one year on commencement of the position

• Ordained and endorsed by denomination.

• Palliative Care Hospice Advanced Certification (BCC-PCHAC) or must obtain BCC-PCHAC board certification within 3 years on commencement of the position.

• Advanced continuing education training and certifications in Spiritual Health, Mental Health, Trauma Informed Care, Cognitively-Based Compassion Training, Compassion-Centered Spiritual Health Training preferred and encouraged.

• Intermediate computer knowledge: Uses Microsoft Word, Excel, Outlook, and Windows.
