We are seeking a(an) ER Clinical Pharmacist


Position Responsibilities:

  1. Responsible for the provision of comprehensive clinical pharmacy services for the ED and all associated areas (eg. Pediatrics, Trauma, Urgent Care) including pharmacokinetic and therapeutic consultation.
  2. Provide Pharmacy review of medication orders prior to administration and ensure safe and effective drug therapy for ED patients.
  3. In coordination with Clinical Manager, prepare formulary documents for new Emergency Department requests, protocols, policies, and order sets and make recommendations regarding place on formulary. Develop guidelines to ensure appropriate use of high-cost and/or specialized drugs used in the ED.
  4. Participate in the development of medication use management programs within this facility including clinical guidelines, critical pathways, disease management, and drug use programs.
  5. Acts as a preceptor to students and/or residents when applicable.
  6. Participate in the professional development and competency of clinical staff. Functions as a role model and as an educational resource for pharmacy staff.
  7. Serve as a resource to the Emergency Department. Provide inservices and lectures to medical staff and other healthcare providers when applicable.
  8. Keeps informed of all local, state and federal laws covering the storage, handling and dispensing of drugs and for interpreting each prescription order to determine that it meets all legal requirements.
  9. Keeps informed of the action, side effects and proper usage of all new drugs as they are made commercially available.
  10. Maintains awareness of contemporary trends in the profession through the professional literature and regular attendance at professional organization meetings, institutes and seminars.
  11. Provides patient specific medication teaching for discharge medications when appropriate.
  12. Focus on cost avoidance and cost savings due to medication use in the Emergency Department.
  13. Assist with medication reconciliation process for patients admitted to facility through the ED.
  14. Assist in obtaining medication allergies and reactions from ED patients.
  15. Proficient in the use of Microsoft Office applications, Meditech or other required operating systems, Order Scanning Systems, Automated Dispensing Units, Packaging Equipment, Physician Order Entry, and Clinical Integration to systems.
  16. Maintain effective communication with Horizon directors and leadership teams on emergency department management opportunities
  17. Work with Staff and clinical pharmacists with dosing and coverage of all patients in the hospital when called upon.
  18. Work with Quality and Pharmacy on Medication reconciliation, Stroke, SCIP, and all Core measures.
  19. Participates in special projects as needed and performs other duties as assigned.

What qualifications you will need:


  • Pharmacist
  • Doctorate Degree
  • Advanced Cardiac Life Spt, or ACLS Instructor must be obtained within 1 year of employment start date
  • Basic Cardiac Life Support, or BCLS Instructor must be obtained within 30 days of employment start date
