Position summary: Incumbent is responsible for delivering social services, including psychosocial evaluation and treatment, advocacy, teaching, referrals to community services, and assisting as needed with discharge planning. Participates in case coverage and other necessary support for other social workers in the Department of Clinical Social Work / Care Coordination. Participates in formal and informal teaching, including supervision of social work graduate students and in-service education for nurses, nursing students, medical residents, fellows, interns, and ancillary staff. Your Role on the Team Essential Job Functions: The identification of essential functions below is not intended to be an exhaustive list of all duties that may be assigned to this position, nor does it restrict the duties which may be assigned to this position if such duties reasonably relate to the position. Provides in or out-patient psychosocial assessments, therapeutic interventions, and community resource support information and referrals for adult and pediatric patients and their families throughout the medical center. Develops and implements appropriate psychosocial interventions that enhance patient and family preparation for and adherence to medical, surgical, and pharmacological regimens. 20% Organizes and facilitate support group(s) as necessary or requested. 10% Enters progress notes on a daily basis for all patients within caseload. Ensures progress notes are accurate, refer to problems identified on the treatment plan, address progress and gains as related to treatment goals, and are written legibly. Document incidents or unusual occurrence at the time that it happens. 15% Effectively collaborates as needed with the patient, family and healthcare team to facilitate appropriate patient transfers. 15% Conducts and documents assessment of patients within caseload for use in developing a master treatment plan. Develops age and disease appropriate care plans in timely manner as prescribed by Program policies and procedures utilizing all assessments and input from treatment team. Reviews and updates treatment plans for clients within caseload on a timely basis. Supports the participation of patients in planning for their care. 15% Acts as an effective liaison to medical staff to ensure continuity and congruity of hospital services in accordance with the plan of care. Effectively coordinates multi-disciplinary referrals. Facilitates effective and efficient hospital response to physician orders and the acquisition of necessary services. Utilizes treatment planning meetings to review problems, progress and discharge plans for all clients within caseload. 15% Accurately coordinates education needs for the discharged patient/family. Teach families and hospital teams about diagnosis-specific community resources and insurances.
