Under general supervision, the psychiatric social worker provides direct social services to patients and their families on the inpatient and outpatient psychiatric services at Saint Francis Memorial Hospital. The psychiatric social worker provides consultative and direct care to patients outside the inpatient psych unit with regard to legal proceedings around involuntary admission and treatment. Provides rapid and comprehensive individual social assessments. Interviews patients and members of their support system in order to identify problems and determine appropriate interventions. Evaluates family, home environmental and financial conditions in planning for the continual an after care plans. Collaborates with community care providers and initiates referrals to community resources. Facilitates psych-social assessments for patients in areas outside psychiatry. Coordinates and participates in treatment planning activities. Facilitates clinical strategies in order to help the patient accomplish identified treatment goals. Provides clinical information to the members of the multidisciplinary treatment team in order to establish an appropriate treatment plan. Provides an initial discharge plan in the patient's individualized treatment care plan on admission to the inpatient psychiatric unit, and revises this plan as needed throughout the course of stay. Investigates and documents probable cause and evidence for continual involuntary admissions, throughout the hospital. Interviews patients and their support system to obtain evidence. Represents in probable cause hearings as needed. Coordinates writ hearings per patient request. Provides teaching and clarification to the patient and family regarding involuntary hold, conservatorship, confidentiality and competency laws, and patient rights and appeals processes. Pursues and coordinates prompt filing of necessary holds and/ or conservatorship petitions to facilitate recommended continued care for patient, offering guidance to Saint Francis Memorial Hospital Social Workers who are employed outside psychiatry. Provides early discharge planning assessment and coordination of discharge services. Interviews the patient and their family/support systems to determine effective treatment and after care plans and to facilitate timely discharges. Collaborates with community and managed care providers to determine after care needs and to advocate for the patient to receive appropriate services in a timely manner. Initiates new community referrals in meeting the patient's after care needs. The final salary and offer components are subject to additional approvals based on UC policy. Your placement within the salary range is dependent on a number of factors including your work experience and internal equity within this position classification at UCSF. For positions that are represented by a labor union, placement within the salary range will be guided by the rules in the collective bargaining agreement. Provides individual patient interventions and family support interventions. Serves as mentor to the multidisciplinary team to assist in the development of clinical strategies. Provides group and family activities and programs. Provides crisis interventions for patient and family, and assault management as needed. Coordinates after care plans with community based agencies. Provides and maintains proper documentation in the patient care record in compliance with CMH, JCAHO, M-Cal, and PSRO Standards. Provides Social Assessment within 72 hours of patient admission and maintains weekly progress notes entry on all patient continual stay beyond 72 hours. Documents all family work, community referrals and final discharge plans. Provides documentation of patient acuity to maintain continual care certification. Carries out and adheres to inpatient psychiatric and Hospital policies and procedures. Maintains meeting of established competencies. Provides staff consultation, support and interpretations of social work practices and procedures. Attends unit staff meetings, policy and program development meetings. Facilitates orientation of new employees and students to the unit. Pursues lifelong learning in areas in expertise. The salary range for this position is $32.09 - $68.87 (Hourly Rate).
