Job Summary and Qualifications

POSITION SUMMARY:

Following physicians' orders, the Pharmacist compounds and dispenses medications and other pharmaceuticals. The Clinical Pharmacist is responsible for providing pharmaceutical care activities to ensure safe and effective drug therapy through the provision of clinical services. The Clinical Pharmacist coordinates and oversees the delivery of pharmaceutical services and provides monitoring and intervention in therapeutics to provide cost effective care with optimum outcomes. The Clinical Pharmacist fulfills departmental responsibilities within the long range plan, mission, vision and values of the hospital. Cares for Neonate/Infant (0-12 months), Pediatrics (1 – 17 years), Adults (18 – 79 years), and Elderly (80+ years).

Position tasks and responsibilities include:


  • Maintains strong knowledge of hospital policies and government regulations and dispenses accordingly.

  • Consults with physicians and nursing staff regarding patient reactions, errors and complaints.

  • Determines and consults on possible drug interactions.

  • Assists in the supervision and training of pharmacy technician personnel.

  • Participates in Drug Utilization Review.

  • Assists with quality improvement initiatives, inventory and ordering of pharmaceuticals.

  • Monitors, records and communicates as appropriate utilizing computerized documentation systems.

  • Utilizes knowledge of age specific needs of patient in performance of duties and responsibilities.

  • Orients and mentors new staff members.



    • Follows Standard Precautions using personal protective equipment as required.

    • Participates in Clinical Monitoring activities as assigned.

    • Participates in maintaining department inventory levels

    • Orders medication based on clinical need in conjunction with formulary initiatives

    • Participates in maintaining department formulary initiatives

    • Communicates with appropriate medical, nursing and allied health professional staff members with regard to non-formulary decisions

    • Works with DOP or designee as needed in formulary clinical review process

    • Provides a safe environment for patient, visitor and employees through identifying, correcting, and reporting health and safety hazards.

    • Accountable for maintaining and improving own job knowledge, skills, and competence.

    · Reviews and interprets all physician orders received, using the patient profile. Screens all medication for clinical appropriateness and intervenes when appropriate:


    o Monitors drug therapy regiment for contraindications, drug-drug interactions, drug-food interactions, allergies, and appropriateness of drug and dose.


    o Renal dosing on appropriate drugs-checks initial dose.


    o Duplicate orders discontinued.


    o Changes IV orders to PO when appropriate.


    o Monitors for incompatibilities, concentration and rate.


    o Pharmacokinetic monitoring for appropriateness.


    · Evaluates subjective and objective responses to drug therapy to determine effectiveness. Checks drug levels when appropriate.


    • Other duties as assigned

    Required Job Qualifications include:


    · Graduate of an accredited school of pharmacy


    · Current Kentucky Pharmacist License


    · One Year of hospital experience preferred



    • Customer service abilities including effective communication and listening skills.

    • Computer skills

    • Critical thinking skills, decisive judgment and the ability to work with minimal supervision in a fast paced environment.

    • Ability to perform work that requires frequent standing, bending, reaching, squatting, kneeling, moving, lifting of patients and/or equipment up to 10 lbs.
