Nurse Navigator - Ambulatory - Military Veterans

In congruence with the holistic care provided by the Division of Nursing and Department of Pulmonary Medicine, the Nurse Navigator functions as a member of the multidisciplinary team as an advocate and educator for patients from point of entry, through diagnostic studies and treatment plan for all Cystic Fibrosis (CF) patients in both the inpatient and ambulatory settings. The Nurse Navigator’s primary function is to build a relationship with patients and team members, to coordinate a plan of care including appointments, transportation, education, provision and /or enablement of support services and representation within the multidisciplinary care environment. The position encompasses the roles of coordinator, liaison, and educator. The Nurse Navigator contributes to the productive and efficient operations of the healthcare system. Facilitates and coordinates quality patient care in a timely and cost-effective manner while promoting multidisciplinary collaboration between all members of the health care team. Requires knowledge of infants to adulthood growth and development. There is potential for collaborate with all other hospital departments as well the CF Foundation.
