Job Type: Officer of Administration Bargaining Unit: Regular/Temporary: Regular End Date if Temporary: Hours Per Week: 35 Standard Work Schedule: Building: Salary Range: $120,000.00 - $135,000.00 The salary of the finalist selected for this role will be set based on a variety of factors, including but not limited to departmental budgets, qualifications, experience, education, licenses, specialty, and training. The above hiring range represents the University's good faith and reasonable estimate of the range of possible compensation at the time of posting. Position Summary The Nurse Practitioner will work collaboratively with Physicians, NP peers, Registered Nurses, Research Coordinators, other members of the multidisciplinary care team, and administrative support staff. The NP will render direct patient care during outpatient office hours including but not limited to, obtaining patients' history, performing comprehensive and targeted physical examinations, ordering and evaluating diagnostic tests, and diagnosing/managing chronic as well as acute patient problems. The NP will also be responsible for counseling/educating patients and their caregivers on disease processes and care management as needed. The provider will be engaged in all aspects of patients' clinical care and work with other members of the care team to provide seamless care across all venues including outpatient, inpatient and clinical research venues. In addition, the Nurse Practitioner will also attend and coordinate Pulmonary Hypertension care at specialty clinics. The NP will also round on the inpatient Pulmonary Hypertension service once/twice a week and coordinate and communicate with the inpatient teams re aspects of Pulmonary Hypertension management. The NP will be expected to collaboratively and autonomously facilitate and deliver care to a specific subset of patients with all forms of Pulmonary Hypertension. The NP will obtain and review outside records/reports as needed and follow-up/collaborate with the Physicians to re-develop the treatment plan as necessary. The NP will work with his/her NP peers and the Registered Nurse team to triage daily patient calls and communicate with referring Providers. The NP will also be expected to work with the multidisciplinary team including but not limited to Social Workers, Nutritionists, Pharmacists, Surgeons, clinical research team and other specialists as necessary to provide/coordinate care across the continuum, including in-patient admissions and clinical research protocol visits. The NP is expected to participate in weekly Team meetings and monthly multidisciplinary meetings. The NP will be required to participate in the on-call schedule with the Pulmonary Hypertension Team, Physicians and NP. Responsibilities The candidate will be responsible for the following job duties: Assists in development, implementation, and evaluation of standards of care and professional practice in collaboration with members of the health care team and in accordance with institutional policies and regulatory agency requirements. Perform and document comprehensive surgical, medical and psychosocial health histories, examinations (document pertinent positive and negative findings), and initiate/maintain a problem/medication list. Order and interpret diagnostic studies including but not limited to laboratory and imaging studies, diagnostic impression, and/or form a differential based on the subjective and objective data obtained. Facilitate hospital admissions, transfers, and discharges as well as coordinate appropriate inpatient and outpatient accommodations and home-care. Coordinates patient care activities, based on established priorities, including teaching, counseling, and discharge planning, and directs/delegates appropriately to members of the health care team. Perform phone triage for acute clinical issues and conduct phone assessments as required per protocol and prescribe treatment as needed. Performs related duties & responsibilities as assigned/requested. Minimum Qualifications Requires Master???s degree or equivalent in education and experience, plus two years of related experience. Preferred Qualifications Pediatric Cardiology inpatient or outpatient experience. 3 years of related NP experience. Computer proficiency with the ability to quickly learn new applications. Possess strong organizational skills and be detail oriented. Other Requirements Current NYS NP License, maintain and renew when necessary. (NP???s are required to have a NYS license to qualify for the position). Current NYS RN License. NP Board Certification (FNP or PNP). DEA License. BCLS and ACLS certification.
