A Geriatric RN Medical Care Manager contributes to AHF by utilizing strong nursing clinical skills and understanding of the Ryan White Program to serve the clients within their area. The Geriatric RNMCM involves working closely with patients living with HIV/AIDS, ages 60 and older, who have multiple psychosocial and clinical health[1]related needs. This will include providing health education, medical coordination, follow-up of medical treatments and patient advocacy. Working directly within the Healthcare Center, you will meet with patients and complete Comprehensive Assessments, develop Care Plans, and provide ongoing education to patients so that they understand their diagnoses and medications, while maintaining adherence goals designed to improve overall health through counseling and skill-building. You will provide advocacy and emotional support for clients to prevent health disparities. You will provide or refer to a licensed medical provider for assessments involving ADL abilities, cognitive functioning, vision, hearing, nutrition, fall prevention, incontinence, Osteoporosis, and medication contraindications.

A Geriatric RN Medical Care Manager enhances the reputation of AHF by utilizing strong core nursing principles. You will meet our patients where they are, identify strengths and opportunities for growth, and assist them with realistic goal setting. With your collaboration and engagement skills, you will bring the essence of the Ryan White Care Act (the safety net for uninsured HIV/AIDS patients) to our most vulnerable population. Beyond the scope of the contract, your work contributes to measures established by the HIV/AIDS Bureau (HAB) and the Department of Health and Human Services.

To be successful as a Geriatric RN Medical Care Manager, it is crucial to ensure that the AHF Core Values and Mission stay top of mind with all that you do. AHF has a collaborative organizational structure where staff are accountable to multiple leaders. The Geriatric RN Medical Care Manager will work as a part of a close-knit team to ensure patients have timely access to comprehensive medical care and social services while promoting and supporting patient independence and self-sufficiency. Also, you will assist patients in developing, achieving, and keeping healthcare appointments, taking ARTs and other prescribed medications as prescribed, complete home visits and communicate with healthcare providers to coordinate care for comorbid or chronic disease conditions. Finally, you will support clients in maintaining adherence goals designed to improve overall health through education, counseling, skill building, and increased access to services within the continuum of care of HIV Health Services in a primary medical care environment.

Licenses and Certifications: Bachelor's degree in Nursing (BSN) with a minimum of 1 year of Case Management experience. Knowledge of Ryan White Services is preferable. Association of Nurses in AIDS Care (ANAC) certification preferred
