INSTITUTE CLINIC The Licensed Clinical Social Worker position is located at The MIND Institute which has on-site Developmental Pediatricians, Psychiatrists & Psychologists co-located and providing a collaborative care delivery model. Institute Clinic City Sacramento Union Representation Yes Benefits Eligible Yes THIS IS NOT AN H1- B OPPORTUNITY Responsibilities The primary role of the LCSW is to assist children and families/caregivers to cope with chronic medical, emotional, and neurodevelopmental disabilities. They will provide support for social/emotional issues and practical arrangements related to the patient's illness, as well as promoting the delivery of effective, cost-efficient, quality patient care. The LCSW delivers age-appropriate social work care to patients and their caregivers in accordance with agency policy and procedure and state and federal regulations. The LCSW serves as an integral member of the healthcare team providing assessments, coordination, treatment planning, information and referral to community resources and other social work services to meet the complex needs of patients and families seen in the clinic settings. The LCSW shall adhere to and promote the ethics and values of the social work profession, using the NASW Code of Ethics as a guide to ethical decision making. The LCSW will advocate for the patient's right to self-determination, confidentiality, access to supportive services and resources, and appropriate inclusion in decision making that affects their health and well-being. The LCSW will be an active participant and work collaboratively with the multidisciplinary team by recognizing and offering services that address the biopsychosocial needs of the patient. The LCSW position requires a high level of clinical judgement, problem-solving skills, creativity, organizational and priority setting abilities, with excellent communication skills The LCSW demonstrates the ability to fulfill these responsibilities through clinical assessment, accurate and sound judgement, and maintaining professional communication. The LCSW may be asked to participate in special projects and collect data to improve the quality of services delivered to our patients. The LCSW must be flexible as changes in technology, healthcare coverage, regulatory requirements, and our patient population may mandate changes to workflow, work standards, and work practices on an as needed basis. Required Qualifications *Applicants are required to upload license and certification if required of the position. * California Licensed Clinical Social Worker (LCSW) Graduation from an accredited school of social work (MSW/LCSW) 3 years minimum of demonstratedcsocial work experience Experience in identifying and analyzing care/service needs for patients Knowledge of Autism, Intellectual Disabilities, and other Neurodevelopmental Disabilities Demonstrated ability to work on a multidisciplinary team Must have solid psychosocial assessment skills Must demonstrate ability to handle demanding workload involving multiple tasks effectively and efficiently Must be able to master detailed and complex information regarding benefits and coordination of care Knowledge of chronic and acute disease and how it influences patient and family functioning Skills to comprehend and assess patient's grievances to quickly locate appropriate resource for assistance Working knowledge of the organization and how to get issues resolved Excellent analytical, interpersonal, and problem-solving skills Extensive, broad-based clinical experience required Skills to perform in-depth chart analysis to provide accurate information in a concise, informative manner Requires a high level of clinical judgment, problem-solving skills, creativity, organizational and priority setting abilities, with excellent communication skills Knowledge of care/services provided in differing levels of care, i.e., ambulatory, and institutional settings Knowledge of community resources Knowledge/experience in assessing patients in a clinic setting and/or telephonically to recognize changes in health/mental/social status Knowledge of biopsychosocial and behavioral health treatment plans Analytical skills to identify and obtain pertinent data from multiple sources, accord each piece of information the appropriate weight and consideration, and reach the appropriate logical conclusions/recommendations Ability to work with all levels of staff, both inter- and intra-departmental to create a cooperative relationship and positive work environment Knowledge of legal requirements and guidelines for providing psychiatric services, including mental status exams and 5150 applications Preferred Qualifications Behavioral Health experience Experience within an acute medical hospital, mental health setting and/or health care clinic Skills, knowledge, and ability working with death/dying/bereavement issues Knowledge of community resources Strong advocacy skills Knowledge of various types of diagnoses and treatment options Special Requirements Variable work locations across the Primary Care Clinics, depending on patient/clinic needs Occasional travel to patients' homes throughout Sacramento and adjacent counties Must have valid driver's license and use personal vehicle in work related activities (mileage reimbursed at University rates) All health practitioners have mandatory reporting responsibilities when an adult (or a minor who meets special circumstances as described in UCDMC Hospital P&P 1408) presents at UCDHS as an outpatient or inpatient from any source, and if a person presents for treatment of injuries related to domestic violence; or if one has knowledge of or observes a child who one knows or reasonably suspects is the victim of child abuse; or if one has knowledge of, or reasonably suspects that an elder or a dependent adult is the victim of elder/dependent adult abuse. Non-health practitioners, who suspect that an individual is a victim of abuse as outlined above, are required to report their suspicions to a health practitioner. All employees have a mandatory responsibility for knowing and complying with the Code of Conduct standards and the Compliance Program.
