Job Summary and Qualifications

The Clinical Rehab Liaison has the opportunity to be a clinical detective who locates inpatient rehab candidates throughout the Nashville market. Your skillset can ensure that patient needs are met from both a medical complexity and therapy standpoint. You will have the ability to use your passion to help patients by facilitating those patients you discover into the rehab unit and ensuring they feel comfortable as they transition from acute to rehab. We are looking for a passionate individual who will work well with other healthcare professionals as we take initiative to better the quality of patients’ lives. 

What qualifications you will need:

  • Associate’s Degree in Nursing or Respiratory Therapy or Bachelor’s Degree in Physical or Occupational Therapy or  Master’s Degree in Speech Therapy required 
  • Current licensure as a Registered Nurse, Speech Therapist, Physical Therapist, Respiratory Therapist or Occupational Therapist required 
  • 2+ years of clinical care experience required 
  • 2+ years of inpatient rehabilitation experience highly preferred 
  • Experience as a clinical evaluator preferred 
  • Certified Registered Rehab Nurse (CRRN) preferred 
  • Basic Life Support (BLS) required 

What you will do in this role:

  • Float to different facilities in the TriStar division as needed for onsite coverage.
  • Participate in Weekend/Holiday rotations and be responsible for meeting rotation/minimum requirements.
  • Assist and coordinate intake and pre-admission screening processes.
  • Document evidence of rehab admission criteria within established timeframe.
  • Perform on-site and off-site clinical evaluations to assist Rehab Medical Director in establishing patient eligibility for admission.
  • Provide information to patient/family/caregiver related to inpatient rehab and program features.
  • Communicate expectations and requirements for participation in the rehab program and conduct or coordinate tours of the rehab unit.
  • Obtain information relating to patient resources and healthcare insurance benefits.
  • Demonstrate clinical knowledge of rehab admission criteria to appropriately identify and qualify rehab patients.
