Care Coordinator - Addiction Medicine | The Ohio State University - Military Veterans

Scope of Position The Ohio State University Wexner Medical Center is committed to enhancing the quality of care our patients receive. System-wide Addiction Services (SWAS) serves the patients of the Wexner Medical Center by creating and driving programs that benefit patients across our Emergency Department, Inpatient and Outpatient programs of the Wexner Medical Center. The Care Coordinator assigned to this program facilitates patients through the health care system to include case management services for patients when presenting with substance use disorder to the ED, hospital, or outpatient settings. The Care Coordinator also provides education & consultation to providers, patients, and families regarding the distinct & specific regulations, criteria, terminology, funding, resources, and third-party payors as they relate to addiction care. Position Summary The Care Coordinator monitors the patient care process, collects data to evaluate and improve addiction care, and collaborates with physicians, social workers, peer supporters, and other health care workers to identify patients who would benefit from the program and coordinate comprehensive assessment, treatment, and follow-up evaluation. The Care Coordinator is a primary educator for the payers, providers, and the public. With a caseload of 50 or more adult patients, the Care Coordinator works with patients and referral staff to ensure stable linkages and minimize barriers to treatment in the community, including residential facilities, skilled nursing facilities, and home-based placements. Care Coordinator assists in discharge planning, including locating in-network providers, making necessary referrals, re-routing any inappropriate or related referrals, and scheduling appointments. The Care Coordinator maintains an awareness of local and regional substance use treatment and harm reduction services and state and national addiction care trends, and proactively responds with initiatives to enhance provision of care and ensure efficient resource utilization. The Care Coordinator works to help providers, staff, and patients identify, bridge, and rectify barriers to levels of care. The Care Coordinator works closely with the Patient Navigator to manage all referrals to addiction medicine, and works with others on the addiction service line to provide cross coverage as needed. Minimum Qualifications For Hire: Bachelors degree required , Masters in Social Work degree preferred. Current LSW , LCDC, or other professional healthcare licensure in State of Ohio required ; Independent license preferred ; 2-5 years recent clinical &/or administrative experience in behavioral health or medical settings preferred ; previous case management experience preferred; knowledge of multiple levels of care , computer data bases, and local resources desired . Ongoing - Maintain licensure(s), credentials and CBLs. Practice within the parameters of the appropriate State of Ohio Professional Licensing Board.
