We are looking for a dedicated Licensed Mental Health Professional III (Clinical Social Worker, Licensed Therapist, or Licensed Professional Counselor) like you to be a part of our team. 

Job Summary and Qualifications

  • You will facilitate high quality, patient-centered care by providing psychiatric treatment services to patients and their families
  • You will perform assessments, psycho-education, process groups, treatment planning, discharge planning and therapeutic counseling support through evidenced based interventions in individual, family, and group therapy
  • You will collaborate with the multidisciplinary team to assess and coordinate patient’s discharge planning needs and readiness for discharge
  • You will evaluate re-admissions for gaps in discharge planning processes and facilitate and participate in Community Education, Activities and Support Groups, as appropriate to scope
  • You will provide education and guidance regarding state commitment processes, abuse reporting requirements, duty to warn and guardianship processes and serve as an educational resource to patients, families, interdisciplinary team members, payer representatives, and other parties as appropriate regarding behavioral services

What qualifications you will need:

  • Master's Degree in Social Work or related field required
  • Experience in mental health or related field required
  • Currently licensed as one of the following professionals in the state(s) of practice, according to law and regulation: Licensed Clinical Social Worker (LCSW), Licensed Professional Counselor (LPC)*, Licensed Marriage and Family Therapist (LMFT)*, Licensed Mental Health Counselor (LMHC)*, Licensed Independent Social Worker (LISW, LICSW)* - *Must be authorized by the state to provide outpatient psychiatry treatment services to Medicare beneficiaries
  • Basic Life Support (BLS) (within 30 days)
  • Crisis Prevention Intervention (within 30 days)
  • No Travel: The job does not require any travel.
