Job Summary and Qualifications

Aids patients, families and significant others to enable them to deal with the psychosocial issues that impact mental illness and to maximize benefits from health care services. Position includes psychosocial assessment, treatment planning as a member of multi-disciplinary treatment team, crisis intervention, discharge planning, referrals to appropriate community resources, evaluation of core measures (HBIPS) and networking with community services providers.

• Basic Cardiac Life Support must be obtained within 30 days of employment start date
• Nonviolent Crisis Intervention must be obtained within 30 days of employment start date
• Other-Fingerprint Required
• License Clinical Social Worker (LCSW)
• Masters Degree

This role requires you to be fully vaccinated for COVID-19 based on local, state and /or federal law or regulations (unless a medical or religious exemption is approved).
